A three-team deal involving the Suns, Grizzlies and Wizards was in place on Friday that would have sent Trevor Ariza to the Wizards. It fell apart after the Suns found out one of the Memphis role players they were to receive wasn’t who they thought it was.
It was reported that in addition to Ariza becoming a Wizards, Memphis would be sending Wayne Selden and MarShon Brooks to Phoenix. The Wizards would send Austin Rivers to the Suns and Kelly Oubre to the Grizzlies.
The reason the deal fell apart was due to which Brooks the Grizzlies were sending to the Suns. Memphis thought MarShon Brooks was the player they were sending and the Suns thought they were receiving Dillon Brooks.
Deal's suddenly in peril. Memphis and Phoenix didn't communicate directly on trade, using Washington as a conduit in coordinating the 3-team deal, sources tell @ZachLowe_NBA and me. Grizzlies believe they were trading MarShon, but somehow Suns believed it was Dillon.

How does this happen in the NBA? Way to go Phoenix. This might explain why you only have five wins on the season.
The Suns and Wizards are still trying to come to an agreement for Ariza. The Lakers, who have been interested in Ariza the whole time, are also trying to put something together to acquire him.
